Prescription & process

How the prescription request, the medical review and providing your prescription work at 420doc.

You fill in a medical questionnaire online and choose the medication you'd like via the platform. A doctor licensed in Germany then reviews your information individually. If the prescription is medically supported, your prescription is digitally signed and sent to the pharmacy you selected. You'll also find it in your dashboard.

The prescription request itself usually only takes a few minutes. In many cases the medical review takes place within about 1 to 3 hours. Depending on workload, how complete your information is or any follow-up questions, it can take longer in individual cases. There is no guaranteed processing time.

No. A cannabis prescription is not issued automatically. Every request is reviewed by a doctor. What matters are your details, possible symptoms, previous treatments, risks and the medical assessment. If a prescription is not appropriate or not justifiable, the request can be declined. If no prescription is issued, we refund the prescription fee in full.

No. Previous findings are optional and not mandatory. You're welcome to upload them, though — they can help the treating doctor with the individual assessment. A request is also possible without previous findings, provided your information allows a medical assessment.

Yes. Existing patients can request a follow-up prescription via the dashboard at any time. Here too, each case is reviewed individually by a doctor — there is no automatic entitlement to a follow-up prescription. Honest feedback on effect and tolerability helps with further treatment planning.

If the medical review is positive, your prescription is sent digitally as an e-prescription to the pharmacy you chose. You'll also find it in your patient dashboard, where you can view or download it.

420doc works exclusively with doctors licensed in Germany. The focus is on medical cannabis, telemedicine and secure digital patient care. You get transparent prices, a personal dashboard, an up-to-date medication plan and a digital cannabis ID. If needed, you can also book a video consultation.

Yes. An existing relationship with a family doctor is not a requirement. However, we recommend involving your family doctor in your treatment in the long term so that all therapies are well coordinated. You should always state interactions and pre-existing conditions honestly in your request.

Information about your symptoms, your medical history, previous treatments, current medication, possible pre-existing conditions and allergies is important. Your age, your weight and, where applicable, pregnancy or breastfeeding also play a role. The more complete your information, the better the medical assessment can be.

If information is unclear or incomplete, the doctor may ask follow-up questions or return the request to you to add details. In some cases a video consultation is also useful. Without sufficient information, a medically responsible prescription is not possible.

Yes. If medical, legal or safety reasons speak against a prescription, the request can be declined. This is part of responsible medical work. In this case you'll receive brief feedback, and the prescription fee will be refunded in full.

If the review is positive, your e-prescription is digitally signed and sent to the selected pharmacy. You receive a notification, your medication plan is updated and the digital cannabis ID is stored in your dashboard. The pharmacy then takes care of shipping or pick-up.

No. Patients without previous experience can also submit a request. For first-time use, reviewing, dosing and advice are especially careful. An honest self-assessment of your experience helps the doctor with the assessment.

On the platform you can choose preferred strains based on live availability. However, the final prescription is always decided by the doctor after a medical review. A specific strain or pharmacy can therefore not be guaranteed.

An honest medical history is the basis of any responsible treatment. Only with correct information can risks, interactions and suitable strains be assessed realistically. False information can not only jeopardise the therapy but can also have legal consequences.

Costs & refunds

Transparent prices, what's included and how refunds work.

The first request is free (€0). A follow-up prescription costs €7.50. This includes the medical review, issuing the prescription if the decision is positive, an up-to-date medication plan and the digital cannabis ID in your dashboard. The medication itself is paid for separately at the pharmacy.

For the medication, the regular pharmacy prices apply — you can see them transparently in the live selection. Optionally, you can add a video call (from €59.99) or the Schengen form for travel (from €29.99; one strain per prescription and certificate). Shipping costs depend on the pharmacy and shipping method.

Yes. If no prescription is issued after the medical review, we refund the prescription fee in full. The refund is made automatically via the original payment method.

The medical invoice is issued by a German private practice in accordance with the German medical fee schedule (GOÄ). There is no direct billing with statutory health insurance.

Direct billing with health insurance is not provided. Privately insured patients may be able to submit their invoice to their insurer — whether and to what extent costs are reimbursed depends on the individual plan. Coverage cannot be guaranteed.

Prices vary depending on strain, manufacturer, quantity and pharmacy. Prices per gram of flower usually fall within a wide range. You can see the exact costs in the live selection on the platform before your prescription request — so you can compare pharmacies and strains at your leisure.

Prices depend on manufacturer, strain, availability, import route, pharmacy margin and legal requirements. Seasonal fluctuations or new batches can also affect the price. The live selection keeps you transparently informed.

The prescription fee is paid online during the request process. You pay for the medication itself separately at the pharmacy — depending on its payment methods, for example online for shipping or on site when picking it up.

A neutral name of the practice or payment provider appears on your bank statement. It does not say “cannabis” or anything similar. Banks can generally see who payments go to — but the medical content cannot be inferred from this.

Yes. You receive all invoices digitally in your dashboard and can download them at any time — both for the medical service and, where available, for the pharmacy order.

No. All prices — prescription fee, optional additional services, pharmacy prices and shipping — are shown transparently before you complete your request. There are no automatic subscriptions or follow-up costs without your express consent.

The prescription fee covers the medical review and issuing the prescription. The pharmacy costs relate to the medication itself and, where applicable, shipping. Both amounts are shown separately and paid separately.

Private prescriptions are not billed directly to statutory health insurance. You initially bear the costs yourself. Privately insured patients can submit the invoice to their insurer — however, reimbursement always depends on the plan.

Yes. On the platform you can see live stock and real-time prices from various pharmacies. This lets you compare pharmacies, strains and shipping options before submitting your request.

Doctors & video consultation

Who treats you and when a video consultation makes sense.

At 420doc you are treated exclusively by doctors licensed in Germany — a modern team focusing on telemedicine and medical cannabis. Who specifically treats you depends on when you make your request and on availability.

Yes. If you'd like to speak to a doctor in person, you can optionally book a video call from €59.99. It is not required for a simple prescription request.

In German, English or Turkish. The request, the questionnaire, your patient area and the written patient information are available in these three languages too. If you're not confident in any of them, have a trusted person help you translate the questionnaire, the patient information and the video consultation. It's important that you understand every question and note exactly, because your answers are the basis of the doctor's decision.

A video consultation makes sense if you have individual questions about the therapy, would like more detailed advice, have several pre-existing conditions or if it's unclear whether treatment is an option for you.

Yes. We make it transparent that your request is reviewed by qualified doctors licensed in Germany. The treating doctors are presented on the platform as soon as the legal and data protection requirements allow.

Yes. Every request is reviewed individually by a doctor licensed in Germany. It is neither an automatic approval nor a purely software-based decision.

Yes. The decision always lies with the treating doctor. Your information, pre-existing conditions, possible interactions, risks and the individual benefit-risk ratio are taken into account. A prescription is only possible if it is medically justifiable.

Yes. If medical reasons speak against a prescription or the information is unclear, a request can be declined. This is part of responsible medicine and protects you. In this case the prescription fee is refunded.

Yes. You can ask follow-up questions about your request or therapy via the dashboard. For more extensive topics, a video consultation is recommended, where the doctor can respond to you directly.

No. Treatment takes place entirely via telemedicine — through the platform and, if needed, by video consultation. A personal visit to a practice is not required.

“Medically reviewed” means that content, recommendations and notes on the platform are checked by licensed doctors and put into a medical context. However, they do not replace individual advice in a specific case.

Medical cannabis is prescription-only. This is not an online purchase but medical treatment with medical responsibility, documentation and traceability. That's why there is no automatic approval and no guaranteed prescription.

If needed, you'll receive feedback asking you to add information or a suggestion to book a video consultation. This ensures that the decision is based on sufficient information.

Pharmacy, delivery & pick-up

Choosing a pharmacy, live stock, shipping and pick-up — everything you need to know about redeeming your prescription.

Yes. You choose medication and pharmacy via the platform based on live stock and real-time prices. After a positive medical review, the digitally signed prescription is sent to the selected pharmacy.

You decide whether the medication should be shipped or whether you pick it up at a local pharmacy. For shipping, depending on the pharmacy, you can choose between standard shipping (e.g. DHL) and express shipping.

As a rule, you'll receive your delivery within 1–3 working days — depending on the pharmacy, shipping method and the time of day you order. A guaranteed delivery time cannot be promised.

Shipping is discreet in neutral packaging. From the outside, it's not possible to tell what's in the parcel. The sender is the pharmacy.

Yes. On the platform you can see the current live stock and real-time prices of various pharmacies — so you can compare and choose at your leisure before submitting your request.

Live stock means that the strains and quantities shown are updated almost in real time from the connected pharmacies' stock. Nevertheless, short-term discrepancies can occur in rare cases, for example if a batch sells out at the same time.

If your preferred strain is not available, you'll be informed. In consultation with the doctor, a comparable alternative can be chosen. A specific strain or batch cannot be guaranteed.

Yes. As long as the prescription has not yet been redeemed, a different pharmacy can be chosen in many cases. However, once the pharmacy is processing the prescription, a change is no longer possible.

Yes, provided the selected pharmacy offers pick-up. This is often the fastest way and avoids shipping costs. Have a valid ID ready when picking up.

Yes. The pharmacy checks the prescription formally and pharmaceutically before releasing the medication. This is required by law and part of the pharmaceutical duty of care.

Delivery times depend on the pharmacy, incoming orders, availability of the strain, shipping method and day of the week. Public holidays and peak times can also extend processing.

Mail-order pharmacies deliver conveniently to your home — usually with a short delivery time. Local pick-up saves shipping costs and is often faster, but requires a trip to the pharmacy. Which option suits you depends on your preferences.

Yes. On the platform you can see the range of strains, live stock and real-time prices of several pharmacies side by side. This lets you compare price, availability and shipping options.

Pharmacies have their own pharmaceutical responsibility and can contact the doctor if something is unclear — for example about strain, dosage or interactions. This is a normal part of care.

Dashboard & documents

Where to find your prescription, invoice, medication plan and digital cannabis ID.

You can find all documents at any time in your personal patient dashboard: your prescriptions, invoices, the current medication plan and the digital cannabis ID.

The digital cannabis ID clearly summarises your ongoing therapy. It is not an official government document, but it can serve as additional proof — for example in everyday life or when travelling — and complements your prescription and medication plan.

The medication plan documents your current prescription and dosage. Together with the current prescription, it is the most important proof — at pharmacy visits, in everyday life or when doctors have questions.

Yes. For travel within the Schengen Area, we offer the Schengen form as an express service from €29.99. It is usually issued within a few hours. You print it out and have it signed by the competent authorities. Important: a Schengen certificate can only cover one strain. Your prescription should therefore contain exactly one strain – if it lists several, first request a new prescription with the strain you're taking with you.

In the dashboard you can always see the current status — from “received” through “in medical review” to “issued” or “declined”. You'll also receive a notification about important status changes.

Yes. Your documents remain available in your dashboard and can be downloaded again at any time — as long as your account is active and no statutory deletion periods apply.

Medical documents are subject to statutory retention obligations and are stored for the corresponding period. You'll find them in the dashboard as long as your account exists. You can request deletion at any time — insofar as legal obligations do not prevent it.

In the dashboard you can see which pharmacy your prescription was sent to, the processing status and, where applicable, the tracking of your delivery.

Yes. You'll receive a confirmation by email and can then see the request with its current status in your dashboard.

You can update basic details such as your address or contact details in the dashboard. Medical information is usually recorded again as part of a new request so that the documentation remains clear.

If a document is missing or appears incorrect, please contact support. In most cases the document can be provided again or corrected promptly.

It makes sense to carry your current prescription, the medication plan and the digital cannabis ID. These documents help to verify your medical prescription — but they do not replace a legal assessment of the individual case.

Medical cannabis & use

Legal framework, forms of administration, effects and what to look out for at the start.

Yes. Since 2017, medical cannabis can be prescribed by doctors in Germany and dispensed by pharmacies. Since 1 April 2024, it is generally no longer prescribed on a narcotics prescription but on a normal prescription or e-prescription. It remains prescription-only.

Medical cannabis is available, among other forms, as dried flowers, extracts or capsules. At 420doc the focus is on prescriptions for inhalation with an approved medical vaporiser.

As with any medication, side effects can occur — such as tiredness, dry mouth, dizziness or slight impairment of concentration. Many patients go through a short getting-used-to phase at the start. Pay particular attention to your body at the beginning and follow the medical advice.

These are rough strain classifications: indica-dominant strains are considered more calming, sativa-dominant strains more activating, and hybrids lie in between. However, what matters is your individual reaction and the medical recommendation — this classification is only a first point of reference.

Terpenes are natural aromatic compounds in the cannabis plant. They shape smell and taste and can influence the effect profile. Which strain suits you is decided by the treating doctor.

THC is the main active ingredient, which can cause psychoactive effects. CBD is not psychoactive and can have other effects. Both occur in different ratios and influence the effect profile. Which combination is suitable depends on the individual case.

No. A higher THC content does not automatically mean better therapy. What matters is a suitable strain, dosage, tolerability and the interplay with other ingredients such as CBD and terpenes. A high THC content can also intensify side effects.

A suitable strain results from the medical assessment, previous experience and tolerability. For first-time use, treatment usually starts with low-dose strains. It's useful to note effects and side effects and discuss them with the doctor.

The effect depends on metabolism, how you feel on the day, dosage, strain, form of use, pre-existing conditions and other medication. That's why two people can experience the same strain very differently. A blanket recommendation is therefore not possible.

Start in a calm environment, with a low dose and without appointments that require full responsiveness. Don't plan any driving, operating machinery or alcohol. Observe effects and side effects and follow the medical advice.

Smoking produces combustion products that can strain the airways and lungs. From a medical point of view, inhalation with an approved vaporiser is therefore generally preferred — also because the dosage is easier to control.

A vaporiser heats the flower below the combustion temperature. The active ingredients are inhaled without the typical harmful substances of combustion. Approved medical vaporisers are intended for medical use.

Yes. Depending on the strain, dosage and how you feel on the day, cannabis can have a tiring, balancing or restless effect. Palpitations, dizziness or feelings of anxiety are also possible in rare cases. If symptoms are significant or persistent, you should seek medical advice.

With mild side effects, it can help to reduce the dose, consider a different strain and document the effects. With stronger or persistent symptoms, please seek medical advice. In acute emergencies, call the emergency services.

A simple therapy diary helps you understand effects, side effects and suitable times of day. This makes it easier for the doctor to adjust the therapy and makes follow-up conversations or follow-up requests more precise.

Driving, travel & everyday life

What you should know about driving, police checks, travel and work — without blanket promises.

A prescription does not automatically mean that you can drive at any time. What matters is your actual fitness to drive. Especially at the beginning, when the dose changes or with side effects, you should be particularly careful and not drive if in doubt. During checks, your prescription, medication plan and pharmacy receipt can be helpful, but they do not replace fitness to drive.

Keep your current prescription and your medication plan to hand — they are the most important proof. The digital cannabis ID can also help, but it is not an official document. Calmly refer to your medical prescription and use as prescribed.

For travel within the Schengen Area, you need your current prescription, your medication plan and ideally the Schengen form. Outside the Schengen Area, rules vary greatly from country to country — find out in good time from the relevant embassy.

During the getting-used-to phase, don't plan any activities that require full responsiveness — such as driving, operating machinery or demanding appointments. Observe how you react to your medication and follow the medical advice.

Fitness to drive means that you are physically and mentally able to take part safely in road traffic. It depends, among other things, on concentration, reaction time, perception and tiredness. Anyone who feels impaired should not drive — regardless of the substance.

A prescription is important proof, but it does not replace an assessment of fitness to drive. In individual cases, the police and authorities may investigate further. Don't use cannabis immediately before driving and talk to your doctor if in doubt.

For road traffic there are legal limits for THC in the blood. Even as a patient with a prescription, a violation can have legal consequences if fitness to drive is impaired. There is no blanket statement that it's “always allowed”. If in doubt, don't drive.

This is possible in most professions, provided you can carry out your tasks safely. For activities with special responsibility — such as operating machinery or safety-related areas — stricter requirements often apply. Talk to your doctor if in doubt and check your employer's internal rules.

Activities with a high level of responsibility require full concentration. Particular caution applies here — especially when the dose changes, with new strains or when tired. A medical assessment and transparent communication with the relevant parties can be useful.

Within the Schengen Area, this is generally possible with the right documents. Outside the Schengen Area, the rules vary greatly — some countries prohibit import without exception. Find out in good time before every trip.

For travel within the Schengen Area with narcotics-relevant quantities, the Schengen form is strongly recommended. We offer it as an express service via the dashboard. It must be signed by the competent authority before travelling. Important: a Schengen certificate can only cover one strain. Your prescription should therefore contain exactly one strain – if it lists several, first request a new prescription with the strain you're taking with you.

Transport your medication in the original pharmacy packaging with its label. Carry your prescription and medication plan with you. This way you can prove the medical prescription at any time.

Doctors advise against this. The combination can intensify effects and side effects, significantly impair responsiveness and increase risks. Anyone who has consumed alcohol should certainly not drive.

Mixed use with alcohol, medication or other substances can trigger unpredictable interactions. The risk of side effects and misjudgements also increases. You should always discuss such combinations with your doctor.

Yes. During the getting-used-to phase it's not clear how your body will react to the therapy. During the first 14 days of treatment and during the 14 days after every dose change, don't drive or operate machinery. Even after that, a fixed waiting time doesn't guarantee that you can drive safely, and your own feeling alone isn't enough: never drive if your attention, ability to react or coordination could be impaired, and never after alcohol or other recreational drugs. Discuss with your doctor whether and when driving is acceptable for you.

Legal situation & legalisation

What has changed for patients since the 2024 cannabis law reform — and what remains clearly separate.

The 2024 reform newly regulated the handling of cannabis for adults. For medical cannabis: it remains prescription-only and, since 1 April 2024, is generally no longer prescribed on a narcotics prescription but on a normal prescription or e-prescription. The medical review and the requirement to obtain it from pharmacies remain in place.

No. Medical cannabis is prescribed by doctors, dispensed in pharmacies and subject to clear quality, documentation and advice standards. Recreational cannabis is subject to different rules. The two areas should be considered clearly separately.

Since 1 April 2024, cannabis flowers and cannabis extracts for medical purposes have been exempt from the German Narcotics Act and are regulated in the Medical Cannabis Act (MedCanG). They remain prescription-only — so a medical prescription is still required.

The Cannabis Act (CanG) mainly regulates the handling of cannabis for adults outside the medical context. The Medical Cannabis Act (MedCanG) regulates medical use — i.e. prescription, dispensing by pharmacies, quality and documentation. Both laws exist in parallel.

For non-medical cannabis, the CanG has its own rules on possession and quantities for adults. These concern the private, non-medical area. Patients with a prescription should always be able to prove their medical quantity with a prescription and medication plan.

Home growing for private consumption is regulated for adults in the CanG under certain conditions. For medical patients, it is not a substitute for medical care: medical cannabis from the pharmacy is standardised, quality-tested and documented — home-grown cannabis is not.

No. Cannabis social clubs are non-medical growing associations with their own rules. They do not replace medical care. You can only get medical cannabis with a doctor's prescription and from a pharmacy — with documented quality and advice.

Pharmacy cannabis is subject to strict quality requirements: defined active ingredient content, verified origin, documented batches and pharmaceutical advice. Black-market products are untested, their ingredients are unclear and cutting agents or contamination are possible. From a medical point of view, this is a considerable difference.

In principle, licensed doctors (with a few exceptions, such as veterinary or dental medicine) may prescribe medical cannabis. Whether they do so in an individual case is a personal medical decision — many doctors do not prescribe it or only to a limited extent.

Medical cannabis is prescription-only because effects, side effects, dosage and interactions have to be assessed by a doctor. The prescription is the basis for safe therapy and for dispensing at the pharmacy.

Legalisation does not change the obligation to be fit to drive. There are legal limits and assessment criteria for road traffic. Patients with a prescription must also assess their fitness to drive on their own responsibility — if in doubt, don't drive.

Medical cannabis and recreational cannabis are clearly separate areas. A prescription does not entitle you across the board to everything the CanG allows for adults. Likewise, legalisation does not replace medical care — prescription, pharmacy and documentation remain central.

They protect you. The medical review assesses benefits and risks individually. Documentation helps with follow-up conversations, questions, pharmacy processing and, where applicable, with checks or travel. Without this basis, treatment would be neither safe nor traceable.

Yes. Laws can be amended — both the Cannabis Act (CanG) and the Medical Cannabis Act (MedCanG). We monitor changes closely and update information as soon as relevant points change. If in doubt, clarify specific legal questions with a professional.
